MISSION
The mission of the St. Anthony Park Block Nurse Program is to enrich the capacity of older neighborhood residents to maintain appropriate levels of independence and interdependence within the community through neighborhood-based professional and volunteer staff working together to integrate health services, social services, education and advocacy.

    CHARITY RESPONSE TO REVIEW:
    St. Anthony Park Block Nurse Program has assisted over 3000 seniors since it began 26 years ago. The program, the first of its kind, has become a model for similar programs across the country. During the past year we have enabled 34 seniors to continue living at home independently by providing nursing and supportive services. Staff and volunteers have provided 240 home nursing care visits, 510 home health aide visits, 518 non-nursing visits, senior exercise classes 5 days per week, and 1325 hours of volunteer service which included transportation for 238 medical visits, grocery shopping, and other errands and approximately 12 home delivered meals each day. SAPBNP also provides supportive services for caregivers of elderly relatives and for friends and advocates of seniors whenever possible.
