Touchstone Mental Health served 780 clients with high quality, person-centered mental health care, assisting 69% of these individuals in attaining an annual physical and collecting critical health care data (MN 10X10) for 57%. Efforts to integrate diabetes management, smoking cessation and wellness practices were available to 100% of our consumers. Consumer Satisfaction Surveys showed that 94% of respondents liked the services they received, 91% were provided helpful information about health and wellness, 91% reported that Touchstone staff believe in their recovery and growth and 95% shared they were treated with dignity and respect. 87% of surveyed clients report experiencing an improvement in their quality of life. Program outcomes show the valuable work of Touchstone Mental Health staff impacts housing (85% retained housing), reduced avoidable hospitalizations (90%), and progress made on client-centered recovery goals (91%). Staff participates in providing training to the community through various statewide associations, and engages in statewide workgroups to support mental health awareness, legislative efforts and statewide healthcare development. Touchstone has been actively developing innovative new programming, including new and affordable housing for 40 individuals, and an adjoining Health and Wellness Center for the greater Twin Cities area due to open in July 2013.
Current Goals
In 2013, Touchstone Mental Health is aligning its’ goals with an updated agency vision statement: “Touchstone Mental Health is an organization of excellence creating innovative, person-centered care with individuals whose lives are affected by mental illness. We transform how services for mental, physical and social health intersect to achieve optimal wellness.” The focus of 2013 goals are continued high quality care, improved outcomes for all key indicators, improved outcome reporting, implementation of new programming at our Health and Wellness Center and Rising Cedar Apartments, offering 24 Hour Customized Living services, improved fundraising and external partnerships (ex. Managed care, Primary Care, etc…). Our vision and goals will also continue to focus on continued innovation and impact on systems of care that allow persons living with mental illness to access services that improve quality of life and wellbeing.
