Accomplishments
Portico screened over 14,000 Minnesotans during 2013. We recieved over 6,105 calls through our Health Care Ascess Line that resulted in screenings. While most of the calls are from the Twin Cities, we received calls from 50 of the 87 Minnesota counties. We made over 5,000 referrals for other needs including food, housing, dental and mental health services referrals to a network of more than 400 nonprofit service providers. We helped 2,300 become enrolled in Minnesota Health Care Programs. We referred over 1,000 people for dental services to over 30 local dental providers. For those screened individuals ineligible for public coverage, Portico Healthnet partners with hospitals and health plans located Dakota, Hennepin, Ramsey, and Washigton counties to offer its own primary and preventive health care access program. Eligibility is based on residency within those four counties, and annual income falling beneath 275 percent of Federal Poverty Guidlelines. Over 1,900 uninsured individuals were participating in the Portico Healthnet coverage program operating at fully funded capacity in Dakota, Hennepin, Ramsey, and portions of Washington counties.
Current Goals
1. Establish Portico’s role as the Metro Area Center of Excellence for Enrollment and other health related referral services. 2. Enhance hospital partners’ efforts to most effectively serve low income and uninsured populations. 3. Enhance effectiveness of communications to target audiences about Portico’s ability to increase health coverage and improve management of health care services for people who are uninsured. 4. Pursue activities that support these goals and lead Portico to more stable funding sources and increased financial stability.
Community or Constituency Served
Outreach and Enrollment: Portico’s Health Access Line serves Minnesotans across the State in need of low-cost health care by providing medical, dental mental health, prescription resources and general community resource referrals. Portico’s certified MNsure navigators assist in enrollment by screening, assisting with MNsure applications and conducting extensive follow up services for uninsured or underinsured consumers inquiring about health care coverage. Portico’s navigators assist consumers in securing alternative coverage or referrals if determined ineligible for MNsure program coverage for any reason. Portico also offers training opportunities for the general public and community organizations interested in learning about the best practices being developed by Portico Outreach and Enrollment, MNsure policies and programs, and developing referral partnerships to extend enrollment opportunities around the State of Minnesota.
Geographic Area Served
Coverage and Care Management: Low-income, uninsured individuals living in our service area (currently Dakota, Hennepin, Ramsey, and Washington counties of Minnesota) who do not qualify for Minnesota Health Care Programs may be eligible for Portico’s coverage and care management services. The program pays for primary and preventive care services, urgent and specialty clinic care, prescription medications, and interpreter services for medical appointments. Care Management coordinators assist individuals in smoothly accessing and navigating the health care system.
