Accomplishments
In Fiscal Year 2010, Portico screened 16,572 Minnesotans to determine their potential eligibility for Minnesota Health Care Programs (MHCP) and other affordable options for health care coverage. Portico assisted 3,835 individuals from 2,397 low-income households with the MHCP application process, resulting in 2,880 enrollments into Medical Assistance, MinnesotaCare, and the former General Assistance Medical Care program. Through its unique model of coverage and care management, Portico enabled 2,284 low-income, uninsured individuals who were not eligible for MHCP to access a continuum of health care, including primary and preventive physician services, specialty physician services, prescription medications, outpatient mental health services, outpatient hospital services, as well as individualized care management services.
